29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 800 Worcester St, Springfield, MA: On May 28, 2019 and at times prior, employees who performed maintenance and servicing actions on machines including the Series 4000 did not utilize the documented energy control procedures. Mechanic and machine operator employees activated the emergency stops, but did not place locks on isolation devices as instructed in the energy control procedures. On May 28, 2019, an employee received a fingertip amputation injury after reaching into a machine with energy sources that were not isolated and locked.
