29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown was not utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. ESTABLISHMENT: The employer did not ensure through providing specific instructions, visual observation and/or other effective means that the Station 100 P3 Testing Equipment, Number 258957 was turned off and locked out prior to two (2) authorized employees were performing tasks, such as (but not limited to) manually cleaning the sensor of the equipment. On or about 05/26/19, while one of employees was performing the above task, the above equipment inadvertently started resulting the employee to sustain a partial amputation of the tip of the left middle finger.
