29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: a) UMC, Inc. at 3002 Peoria Street in Aurora, CO 80010: On or before May 3, 2019, the employer did not ensure that forklift attachments had been approved by the manufacturer prior to use. Employees of UMC Inc. used a wooden materials hoisting basket built by another subcontractor on location to hoist equipment to various levels of an apartment complex. The employer did not verify that the forklift attachment was an approved forklift attachment for that particular Genie GTH-844 forklift. This exposed employees to struck by and fall hazards while working with the basket from heights greater than 10 feet.
