29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA 300 or equivalent), was not completed in the detail as required by the regulation: 29 CFR 1904.29(b)(3): 29 CFR 1904.29(b)(3): The employer did not enter the number of days away, job transfer or restriction on the OSHA 300 log within 7 calendar days of receiving information. COMPANY-WIDE: On or about and at times prior to May 9, 2019, entries on the 2018 and 2019 Form 300s were either incorrect or incomplete thereby materially affecting OSHA's ability to assess the health and safety of the facility in that: 1. An entry on the 2019 Form 300 for an injury that occurred on March 21, 2019 had not been completed. Column "H" was checked indicating that the employee had missed time from work but column "K" indicated no days away from work. 2. An entry on the 2018 Form 300 for an injury that occurred on December 25, 2018 was incorrectly recorded. Columns "K" and "L" indicated that the employee was out of work for seven days and on job transfer/restricted duty for an additional 18 days; however, a mark was made in column "I" indicating that the employee was only on job transfer or restricted duty.
