29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) On or about 5/7/2019 - 1203 1st Ave SE, Graysville, AL, employees were exposed to caught-in and struck-by hazards when an overall energy control program was not established to control potentially hazardous energy when employees were servicing and/or repairing equipment such as, but not limited to work trucks, aerial trucks, refrigerated liquid trucks, tractors, trailers, telehandlers, and other heavy equipment in the shop area, at customer sites, and during road service calls.
