29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: Such activities were not performed only by experienced and trained employees selected for such work by the competent person. This condition was observed on or about April 9, 2019. a) 1404 East Palmer St., Philadelphia, PA 19125 - The employer did not ensure that scaffolding, on which employees were performing brick masonry at heights up to 24 feet, was built by direction of a competent person qualified in scaffold erection, thus exposing employees to fall from height, struck-by, and contact with electrical hazards. This condition was observed on or about April 9, 2019. Vyacheslav Leshko aka Slavik Leshko was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1218454, citation number 2, item number 7 and was affirmed as a final order on October 11, 2017, with respect to a workplace located at 827 North 26th Street, Philadelphia, PA 19130. Vyacheslav Leshko dba T&S Masonry, LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 963773, citation number 1, item number 3b and was affirmed as a final order on 21 APRIL 2014 with respect to a workplace located at 1532 Manton Street, Philadelphia, PA 19146.
