29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6): a) 1404 East Palmer St., Philadelphia, PA 19125 - The employer did not ensure that the metal fabricated frame scaffold tubular welded scaffolding was constructed at least 3 feet away from electric power lines energized to 120/240 volts., thus exposing employees to contact with electrical hazards. This condition was observed on or about April 9, 2019. Vyacheslav Leshko aka Slavik Leshko was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1218454, citation number 2, item number 5 and was affirmed as a final order on October 11, 2017, with respect to a workplace located at 827 North 26th Street, Philadelphia, PA 19130. Vyacheslav Leshko dba T&S Masonry, LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 963773, citation number 1, item number 1 and was affirmed as a final order on 21 APRIL 2014 with respect to a workplace located at 1532 Manton Street, Philadelphia, PA 19146.
