29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) 1404 East Palmer St., Philadelphia, PA 19125 - The employer did not ensure that employees had donned protective helmets under and around a fabricated frame scaffold and was in danger of being struck by falling equipment and materials, thus exposing employees to struck-by hazards. This condition was observed on or about April 9, 2019. Vyacheslav Leshko aka Slavik Leshko was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1218454, citation number 2, item number 1 and was affirmed as a final order on October 11, 2017, with respect to a workplace located at 827 North 26th Street, Philadelphia, PA 19130.
