29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about April 23, 2019, employees were exposed to injuries from moving parts and pressurized oil in that the thermal press #2 was not deenergized prior to performing servicing tasks such as, but not limited to, filling the Bush Vacuum Pump with oil. The employer did not ensure that energy control application steps were implemented to control the machine's hazardous energy sources prior to the performance of servicing tasks to include: � Ensure that equipment was turned off and shut down. � Physically engage energy isolating devices to control the energy source(s). � Verify that isolation and de-energization of the machine or equipment had been accomplished.
