Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from loose and sliding logs that were cradled in the clamshell grapples of the Ceres Environmental Services grapple trucks, in that the employer did not provide log grapple claw attachments for handling log: a. On or about April 12, 2019, on Sand Ridge Church Rd, a grapple truck operator and his flagger were exposed to struck-by hazards while raising and handling logs that were loosely cradled inside a clamshell grapple attachment in that he equipment and handling practice were not suitable for the task and not authorized by the grapple equipment manufacturer.
