29 CFR 1915.56(c)(3): When a structure or pipeline was employed as a ground return circuit, it was not determined that The required electrical contact existed at all joints. a) USNS 1st Lt. Jack Lummus, Starboard side Life Boat Davit #6: On or about April 3, 2019 an employee was working on the davit without a proper grounding circuit, exposing the employee to the hazards of the generation of an arc. The electrical arc caused the wire cable holding the davit to break, allowing the davit to deploy forward.
