29 CFR 1910.23(b)(10): (A) The employer did not ensure that any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with �1910.145 and removed from service until repaired in accordance with �1910.22(d), or replaced On or about 2/20/19, employees were utilizing extension and step ladders, including but not limited to the Louisville 24' extension ladder, to gain access to parts of machinery during maintenance. Foreign material (grease, oil, and machine coolant) falls from the machinery above onto the floor in the basement and is transferred to the ladders from employees during use. The ladder had a bent shoe pad that was covered in foreign material, and a crack on the slide rail. The ladders were not removed from service when defects were found.
