29 CFR 1910.23(b)(9): Ladders were not inspected before the initial use in each work shift, or more frequently as necessary, to identify any visible defects that could cause injury: On or about 2/20/19, the employer failed to ensure that ladders were inspected for defects prior to every shift they are utilized. Ladders, including but not limited to the 24' Louisville extension ladder, was covered in foreign material (grease, oil, and machine coolant) which had been dripping from the mechanical power presses above, and transferred from employees during use. The ladder also had a bent shoe pad that was covered in foreign material and the slide rail had a crack that was approximately 2.5" in length.
