29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Including but not limited to; a) On or about December 14, 2018 the employer failed to ensure that effective machine guarding was in place on the #11 rotational mold machine. Employees are exposed to a pinch point between the movement of the mold and the platform. The equipment is equipped with two hand control devices to begin the cycle, however employees are able to reach the hazard area before the mold has exited the area. b) On or about December 21, 2018, the employer failed to ensure that effective machine guarding was in place on the #11 rotational mold machine. Employees are able to reach the path of movement of the mold without effective guarding in place. A simple chain link rope was placed in front of the hazardous area including the oven area. c) b) On or about December 21, 2018, the employer failed to ensure that effective machine guarding was in place on the #1 rotational mold machine. Employees are able to reach the path of movement of the mold without effective guarding in place. A simple chain link rope was placed in front of the hazardous area including the oven area. d) On or about December 21, 2018, the employer failed to ensure that effective machine guarding was in place on the #2 rotational mold machine.
