29 CFR 1926.454(b)(2): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining or inspecting trained by a competent person in the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question. This violation occurred on December 18, 2018 at the 115 E. 5th St., Tulsa, Oklahoma worksite: Employees involved in the dismantling of a transport platform system were not trained in the additional precautions necessary when the initial tie in was higher than the maximum specified by the manufacturer exposing employees to the hazards of falls.
