29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) GB Manufacturing Company, on or about December 18th, 2018 at the address of 100 Adams Street, Delta, OH: The employer did not ensure that the energy control program was applied and done in the sequence required. The employer did not ensure that the energy control program was implemented in order to protect employees from the hazards of energized equipment such as but not limited to the spot welders. Employees performing set up activities, which involves the changing of the electrodes, without the benefit of energy isolation were were exposed to pinch point hazards at the point of operation.
