29 CFR 1910.147(c)(4)(ii)(B): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to , the following: Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a) On or about November 28, 2018, the Lockout / Tag Out Instructions for the Redline Seam Welder (A20295), dated October 17, 2018, did not include instructions for removing pneumatic pressure from the sheet pusher air cylinder when a jammed metal sheet prevents the sheet pusher from extending completely and returning to the home position. A check valve prevents the air cylinder pneumatic pressure from bleeding off when the Main Air Supply is shut off.
