29 CFR 1926.353(b)(1): General mechanical or local exhaust ventilation meeting the requirements of paragraph (a) of 29 CFR 1926.353 was not provided where welding, cutting, or heating was performed in a confined space and employees were not otherwise protected: The employer is failing to protect employees from respiratory hazards in the workplace. Employees were welding inside a casing pipe which was 80 feet long and 54 inches in diameter with no mechanical means of ventilation provided to move fumes through the piping. This was most recently documented at the intersection of 144th St. and Dewey St. Omaha, Nebraska on or about November 20th 2018.
