29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section: The employer is failing to protect employees from caught in, struck by, and amputation hazards by not providing specific LOTO procedures for machinery that has multiple energy sources. This was most recently observed at 5000 Rhino Rd, South Sioux City, on November 8, 2018 when maintenance was being performed on a Forel Glass Machine and specific procedures to control hazardous energy were not provided to employees.
