29 CFR 1926.405(b)(1): Openings in boxes, cabinets, and fittings through which conductors entered were not effectively closed: At a worksite located 3119 to 3123 (Shankman Building) and 3125-3133 (Michaelson Building), Kansas City, MO 64109: Employees were exposed to electrical shock hazards. a metal receptacle box was used as a power source for a bench grinder. The box had an opening in the side in which a conductor entered to supply power to the receptacles and had not been closed to prevent unintentional contact with the live conductor.
