29 CFR 1910.1200(h)(3)(i): Employee training did not include methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.): a) Element Services, LLC at Khaki Campbell East, Hwy 14 and WCR 85, Briggsdale, CO 80611: On and before October 27, 2018, Element Services, LLC did not include employee training methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area. The employer moved two 500 barrel tanks containing residual hydrocarbons onto the site for use in a new tank battery. Employees were involved with setting up and connecting pipes to the tanks, and replacing gaskets on lower access panels. The supervisor and two foreman were each provided with 4 gas monitors for use at the site. A review of the data from these monitors showed that two devices had not been turned on for several days. The third monitor was in use, but was not bump tested properly. The employees (not the supervisor or foremen) who were actually removing the lower access panels were not provided with, and were not wearing, 4 gas monitors. Employee's were not adequately trained on the use, function, and purpose of the 4 gas monitors, and on the methods that may be used to detect the presence or release of a flammable hydrocarbons. This condition exposed employees to atmospheric, fire, and explosion hazards.
