29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. a. On or about October 13, 2018, located in the INF Department at Amsted Rail in Groveport, Ohio, the employer did not ensure employees were protected from machine caught-in hazards while performing a group lockout/tagout activity when removing roller pins from FR-64, a pneumatic upender. Three employees were involved in the pin removal without their locks affixed to energy isolating devices.
