Text For Citation: 01 Item/Group: 001A Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about October 1, 2018, the employer did not ensure employees were protected from unexpected energization, startup,or release of stored energy prior to servicing, set up, and maintenance of equipment, through performing the following sequence of actions: 1) Preparation for shutdown; 2) Machine or equipment shutdown; 3) Machine or equipment isolation; 4) Lockout or tagout device application; 5) Release of stored energy; 6) Verification of isolation. Employees engaged in the activity of changing a temperature control valve on a vaporizer located on a customer's worksite located at 4536 IL Highway 251, Mendota, IL 61342 were exposed to explosion, fire and burn hazards from the unexpected release of stored propane that ignited. Abatement certification or documentation is not required for this item.
