29 CFR 1926.964(c)(2): The employer did not determine existing conditions of the electrical lines and equipment that are related to safety of the work to be performed before employees used the live-line barehand technique on energized high-voltage conductors or parts. a) On September 12, 2018? at 211 Greenville Road} sub-station, Greenville AL: Before live-line barehand work began, the employer failed to ensure supervision determined the existing characteristics and conditions. Before connecting the temporary shoefly lines, the employer failed to identify and address the hazards associated with the switches that were opened and cut and the creation of different loads when working with two different feeds, exposing employees to electrical hazards.
