Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: 29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. On or about September 18, 2018, employees worked on air handling unit 18 and did not shut down air handling unit 13 as part of the energy control procedure for activities covered by this part. HVAC maintenance technicians performed troubleshooting, belt inspection and replacement of parts without having shut down the electrical power supply to air handling unit 13 when working on air handling unit 18. On September 10, 2018, an employee suffered a partial amputation while performing activities covered by this part during troubleshooting of air handling unit 18.
