Text For Citation: 01 Item/Group: 001C Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): a. On or about September 13, 2018, the employer did not ensure that employees implemented energy control procedures prior to performing set up activities on the Ideal FSD 074 Fenceline resistance welder. Covered activities included, but were not limited to, wire change overs. An unexpected movement of the welder's line wire magazine exposed an employee to a caught in hazard. Energy control procedures, such as isolation of energy sources from the machine [1910.147(d)(3)], the application of a lock-out device to control each energy source [1910.147(d)(4)], to ensure all residual energy had been removed [1910.147(d)(5)(i)], and verification steps [1910.147(d)(6)], were not implemented to protect employees from machine servicing hazards.
