29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a. 2000 Industrial Highway, Building 5, Eddystone, PA 19022 - The employers Written Lockout Tag-out program did not specifically outline the steps to properly secure raised vehicles to prevent employees from exposure to gravitational energy when employees perform service and maintenance under vehicles. Employees were exposed to fatal crushing hazards. This condition was observed on or about September 5, 2018.
