29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: Green Plains, Inc. - Central City, Nebraska - Employer is failing to ensure that employee(s) shut down or isolate hazardous energy using the specific procedures for the equipment before a servicing operation. On or about August 23, 2018, an employee removing a level puck on Evaporator #13, was severely burned by hot process condensate that had not been drained from the tank.
