Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Industrial Machining Services, Inc. (IMS) at the address of 216 Southgate Drive in Minster, Ohio; On or about August 22, 2018, the employer did not ensure energy isolating procedures were implemented prior to performing service and maintenance activities in Danone North Americas plant. The employer did not: 1) Physically operate energy isolating devices to control energy sources and affix lockout device(s). 2) Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. 3) Verify that isolation and de-energization of the machine or equipment had been accomplished. Employees replacing the cluster valve body of valve 282AV1342 were exposed to hazards associated with high temperature liquids in that valve 282AV1230 was not locked out. An employee suffered second and third degree burns to their arms, legs and mid-section when valve 282AV1230 was pulsed upstream and emptied into the drain where the employee was working.
