29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: On or about August 9, 2018 and times prior, the employer did not conduct and document monthly inspections of approximately 18 cranes/hoists in the South Plant and about 35 cranes/hoists in the North Plant, exposing employees to struck-by hazards.
