29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation no longer exists. Sulphur Unit - On or about July 4, 2018, the employer did not ensure that a 4 inch 300# gate valve on equipment 77H-103 (hydrogen block valve) was bled out at the line opening to depressurized the line. Employees were exposed to a flash fire/explosion hazard.
