Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: a. On or about July 14, 2018, an employee sustained an amputation injury while operating the ALLSTEEL Press Brake, model No. 250-12, where the employer did not provide any form of guarding to prevent the operator from gaining access to the pinch point created between dies at the point of operation. b. On or about July 23, 2018, employee(s) operating the ERMAKSAN Power-Bend Falcon #3 were exposed to crushing hazards due to a maladjusted "laser safety system" that allowed operator(s) to place hands/fingers in the danger zone(s) during operating cycles. The employer did not provide other of guarding to prevent the operator from gaining access to the pinch point created between dies at the point of operation. Abatement documentation is required in accordance with 29 CFR 1903.19(d).
