29 CFR 1926.452(w)(6)(i): Employee(s) were allowed to ride on scaffolds when the surface on which the scaffold was moved was not within 3 degrees of level, and/or not free of pits, holes, and obstructions: (a) Turn Key General Contractors, Inc. at 6305 E Hampden Ave #102, Denver, CO 80222: On July 16, 2018 the employer did not ensure that the mobile scaffold was only used on a flat stable surface. The presence of utility connections and related holes in the cement flooring created a fall hazard when the mobile scaffold was wheeled over a large hole in the cement flooring. This condition caused the wheeled scaffold to tip over, resulting in employee injury.
