29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control device(s): Facility: The employees, as "authorized, affected and other" types were not provided with training and information in the controlling of potentially hazardous and unexpected release of energy sources (lo/to) during servicing and maintenance of equipment, such as (but not limited to) the Entwistle, Rite-size, corrugated box machine. On or about July 3, 2018, an employee sustained an amputation of two fingers on his right hand on a chain and sprocket while reaching inside the machine to clear a piece of cardboard that was jammed in the machine without locking out the machine. The employee was able to reach into an area where a chain and sprocket were only partially covered by a guard.
