29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: (a) ContractOne, Inc. at 105 Base Camp Circle, Granby, CO 80446: On or before June 14, 2018, the employer did not ensure that daily inspections of the north trench were conducted by a competent person. The employer identified hazards of the trench, including water accumulation, trench wall fissures, sloughing of soil, cave-ins, and spoil pile hazards, but did not take prompt corrective measures to mitigate or eliminate these hazards. Employees were working in this trench up to 8 feet deep installing a water line. This condition exposed employees to cave-in hazards and to the hazards of being struck-by falling materials.
