29 CFR 1910.178(l)(2)(iii): All operator training and evaluation was not conducted by persons who had the knowledge, training, and experience to train powered industrial truck operators and evaluate their performance: On or about June 02, 2018, at 7125 NW 6th Avenue, Miami Florida, the employer failed to ensure Powered Industrial Truck operator instruction, training and evaluation was conducted by a person who had the necessary knowledge, training, and experience, during which employees were operating the truck with front-end attachments to lift and move oversized and non-palletized loads throughout a cargo handling, storage, and distribution facility, therefore exposing the employees to struck-by hazards. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
