29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about June 02, 2018, at 7125 NW 6th Avenue, Miami Florida, the employer failed to mark a Powered Industrial Truck to identify a telescoping boom with stone slab clamp; failed to mark the fork extensions (front-end attachments); failed to mark the truck to show the approximate weight of the truck and attachment combination; and the load capacity of the truck and attachment combination at maximum elevation with the load laterally centered, while employees operated the truck with the use of the fork extensions (front-end attachments) to move and lift stone slabs, oversized and non-palletized loads, thus exposing employees to struck-by hazards. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
