Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught between hazards created by the pinch point of the sweep/packer panel and slide and the hopper sill during the execution of the pack cycle on a Pak-Mor 30 yard 60/40 split rear loader refuse compactor. On 6/19/2018, and at times prior, on Truck #33, operating in and around Shrewsbury, MA 01545 waste management workers assigned to, and working on, this truck were exposed to caught between hazards created by the pinch point of the sweep/packer panel and the hopper sill during the execution of the pack cycle on a Pak-Mor 30 yard 60/40 split rear loader refuse compactor. Among other methods, one feasible and acceptable means of abatement includes, but is not limited to: (a) Operate the sweep/packer panel and slide in accordance with Page OP 6-020 of Pak-Mor's (i.e. the manufacturer's) Operation, Maintenance and Parts manual which requires that the employer ensure "no one is standing on the riding steps while the packer panel is in motion". (b) Develop and implement a training program that complies with the 2012 edition of the Pak-Mor Operation, Maintenance and Parts manual as described on page OP 3-010 (and ANSI Z245.1-2012, clause 6.2).
