29 CFR 1910.269(a)(4)(i): The nominal voltages of electric lines and equipment was not determined before work on or near the lines or equipment is started: a) Kearny #12 gas compressor MCC The nominal voltage of lines located inside Siemens Motor Control Center (MCC) were not determined before trouble shooting of equipment was started. On 5/29/18, an employee removed a bolted panel and tested a high voltage line (4160 V) before knowing the nominal voltage, causing an arc flash. Condition occurred on or about 5/29/2018
