29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: On or about May 18, 2018, workers were not instructed to recognize the 39.37 foot area of reduced visibility around motor graders, exposing them to the hazards of being struck by a motor grader: a. Working within the 39.37 foot restricted area of a moving motor grader. Ref: page 40 of the Motor Grader operation manual. b. Directing traffic while standing between the motor grader and the newly dumped dirt was places worker in the direct path of a motor grader and in the motor grader operator's blind spot. Abatement note: 1) Train workers to stay outside of the 39.37 foot restricted area of moving motor graders. Ref: pages 39-40 of the Motor Grader operation manual.
