Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) - On or about 05/10/18 and at times prior thereto employees performed maintenance and repairs of equipment including (but not limited to) the shaker, wash line components, air compressor, and additional machinery subjected to lockout/tagout as described in the equipment inventory of date 05/10/18. The employer applied a single general lockout/tagout procedure, but did not develop and implement a specific written lockout/tagout procedure for each machine. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
