29 CFR 1910.120(q)(2)(ii): The employer did not develop an emergency response plan which addressed personnel roles, lines of authority, training and communication. This violation was most recently observed on April 19, 2018, in the North Plant at the Valero Texas City Refinery when employees in a unit adjacent to the unit experiencing the emergency remained in the process unit, after mustering, to conduct work without first confirming with management what work needed to be conducted and when.
