29 CFR 1926.454(b)(3): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question in which the training included the design criteria, maximum intended load-carrying capacity and intended use of the scaffold: On or about 04/16/2018, in the EDC Unit at Westlake Chemical, the employer did not ensure that each employee who was involved in dismantling a scaffold with an eccentric load applied to it by a cantilevered work platform was adequately trained on the design criteria, maximum load-carrying capacity, and the manner in which to apply that load in order to recognize the hazards associated with the work.
