29 CFR 1926.454(b)(2): The employer did not have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question in which the training included the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question: On or about 04/16/2018, in the EDC Unit at Westlake Chemical, the employer did not ensure that each employee dismantling the scaffold was adequately trained on the correct procedures for disassembling a scaffold with an eccentric load applied to it by a cantilevered work platform in order to recognize the hazards associated with the work.
