29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s). (a) Fabrication Department - The employer did not ensure that employees operating the Accrupress hydraulic press brake were protected from point of operation hazards resulting in one employee losing 4 fingers on his left hand, one finger on his right hand and three crushed fingers on his right hand, on or about April 20, 2018. (b) Fabrication Department - The employer did not ensure that employees operating the Donewell hydraulic press brake were protected from point of operation hazards resulting in potential amputations or crushing injuries, on or about April 20, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
