29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: 1) 13th Street Bridge and Front Avenue: On or about April 11, 2018, the employer exposed employees to fall hazards, while working approximately 9 bucks high on the scaffold system without end-rails installed on all open sides of the scaffolding system.
