29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6): A) Newman Technology, Inc., located at 100 Cairns Road, Mansfield, OH 44903: On or about April 4, 2018, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d) and required for the control of the hazardous energy sources such as, but not limited to, hydraulic were followed while employees conducted activities such as, but not limited to, making adjustments to the equipment such as, but not limited to, the Pipe Bender # 13 located at the 1130 TLA Tank Mount Line in order to adjust the bend shape of the pipes. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled while conducting work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from amputation hazards while employees conducted activities such as, but not limited to, making adjustments to the equipment such as, but not limited to, the Pipe Bender # 13 located at the 1130 TLA Tank Mount Line in order to adjust the bend shape of the pipes.
