29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation and rotating parts. On or about 3/6/18 in the Machine Shop; the employer allowed employees to operate the Amada Hydraulic Press Brake and the Ellis Mitre Band Saw (model 1600), and the Brake did not have a point of operation guard or device, and the unused portion of the band saw blade was not guarded; thus exposing employees to amputation hazards.
