29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: The employer did not protect employees from the hazardous energy of machinery or equipment by turning off or shutting down machinery or equipment as required by energy control procedures, ensuring energy isolation devices were operated to isolate machinery or equipment from their energy source(s), and affixing lockout devices to energy isolation devices by authorized employees. a. On or about February 14, 2018, located in the Machining Department on Rod Line #3 at the Hex Machine, the employer did not ensure that employees implemented energy control procedures prior to entering the confines of the Hex Machine where a pinch point was present while employees were engaged in the positioning of a hydraulic ram for troubleshooting reasons. The machine was not isolated from energy sources prior to engaging in this service and maintenance activity. On February 14, 2018, an authorized employee was troubleshooting the Hex Machine by positioning a hydraulic ram when the operator reached into the confines of the machine to make an adjustment to an adjuster nut. The authorized employee cycled the ram creating a pinch point causing a finger amputation. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from machine servicing hazards.
