29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: A) Toledo Molding & Die, Inc., located at 515 East Gypsy Lane Road, Bowling Green, OH 43402: On or about February 7, 2018, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the Cincinnati Milacron hydraulic deflash press BDP03 located next to the Press # 42. Each employee, who was setting up the 9015 deflash die inside the Cincinnati Milacron hydraulic deflash press BDP03, did not affix a personal lockout or tagout device to the main electrical disconnect of the deflash press before conducting the work.
